(Check one and write a letter including details of the reason for the request.  Proper documentation to support your letter must be attached to your request!)

Illness of student or close family member affecting your academic performance

Special circumstances as determined by the director

Submit documentation of circumstances that caused the ineligibility of your financial aid. Documentation can be a personal letter with physician documentation, two notarized letters from friends or relatives detailing circumstances. Please allow a minimum of five days for processing. You will be notified by mail if further documentation is necessary. You will also be notified by mail of the determination. INCOMPLETE REQUESTS WILL NOT BE CONSIDERED.
